If the service that you need is not on this list, don't hesitate to ask. We will evaluate each request on an individual basis and work hard to develop a plan that will meet your needs.


Help at Home is an innovative program that bridges the gap in healthcare costs for people who do not qualify for government assistance, yet do not have the resources to pay for the entire cost of services they need.


Clients of the Help at Home program will pay for services based on a sliding scale fee. This allows our clients to pay for services at an affordable rate, while charitable donations made to the program will subsidize the remainder of the cost.

Who Qualifies?


The Help at Home program is designed to meet the needs of individuals who are at or below 350% of the federal poverty level and have a need for home care service due to any of the following:


An infirmity or disability due to aging.

A physical/mental disability and/or degenerative medical diagnosis.

A developmental disability or traumatic brain injury.




Getting Started!


Getting started is easy! One phone call or e-mail is all it takes to join our Help at Home program.


Step 1: 

Call us TOLL FREE at 855-CC-IS-4-US (855-224-7487) and ask for the Help At Home program


E-mail us at helpathome@claritycare.org and tell us you are interested in the Help at Home program.

Be sure to include your contact information.


Step 2:

We will develop a customized plan to suit your needs.


Step 3:

Once the plan is agreed upon, a sliding scale fee sytem will assess the cost of service.

The service contract will be signed with the agreed upon rate.

Services will be billed on a monthly basis and payment is due upon receipt.


Step 4:

Service begins. It's really that easy!
