Self-Directed Support Services

Getting Started Is Easy!

Step 1:
Call us toll-free at (920) 236-6560 ext. 1369 or (920) 469-5240, ext. 2350, or send an email to information@claritycare.org and indicate your interest in self-directed support services.  Be sure to include your contact information and any questions you have.

 

Step 2:

We will work with you to set up an appointment to discuss your current situation and how we can work together to develop a plan that meets your needs, based on your schedule and desires.

 

Step 3:

Once the plan is agreed upon, the cost of services will be reviewed, and you will be on your path to greater independence.


Self-Directed Support Services

 

As part of Wisconsin's long-term care system elderly persons and individuals with physical or developmental disabilities can participate in self-directed support services, determining the providers and level of care they feel would best meet their needs. Self-directed support is available to consumers participating in Family Care or IRIS.

 

In defining self-directed support, the State of Wisconsin states that "Each person may arrange for, manage and monitor his or her family care benefit directly, or with the assistance of another person."

 

Clarity Care's self-directed support services include:

 

  • Supportive Home Care Services - homemaker companions offer assistance with tasks such as meal preparation, grocery shopping, housekeeping, laundry, transportation, and more, to enable the client to live more safely and independently in their own home.

  • Personal Care Services - assistance with bathing, toileting, oral care, dressing, and transferring.

  • Residential Programs - direct support providers available in homes for adults with developmental and/or physical disabilties, traumatic brain injuries (TBI), or chronic mental illness.

  • Community Supported Living Programs - provides customized, flexible support to individuals in their own residence who require assistance to live as independently as possible.

  • Day Services - located in Green Bay, this center offers daily living skills training, socialization, and community inclusive activities.

  • Community Employment Services - uses a collaborative approach with care managers and employers in Brown, Fond du Lac, Green Lake, Outagamie and Winnebago Counties, to match individuals with jobs that fit their abilities, qualifications and interests.

  • Caregiver Training - skills training and ongoing education for caregivers on topics such as transfers, client rights, and difficult behaviors.
