Lead Direct Support Providers
Clarity Care is seeking qualified individuals to work full time in our residential programs with adults that have Developmental Disabilities.

 

Lead Direct Support Providers have the following responsibilities/duties:

  1. Assist in performing duties relating to the quality provision of care, supports, welfare, supervision and training of persons with developmental disabilities and/or acute, chronic or long-term.
  2. Assist Supports Coordinator with Person Centered Plans and Support Services. 
  3. Perform activities of daily living and personal cares for consumers.
  4. Transport, escort and assist consumers in constructive social and recreational activities.
  5. Assist in protecting the rights of the consumers.  Assist/teach how to achieve hopes, dreams, goals and objectives through self-direction, decision-making, informed choice and goal setting.
  6. Responsible for grocery shopping and implementation of general nutritional guidelines, menus, and individual dietary restrictions, needs and preferences.
  7. Supervise and assist in medication administration. Ensure Physician’s Plan of Care is carried out. 
  8. Ensure medical, dental and psychiatric appointments are completed in a timely manner.
  9. Prepare for and respond promptly and competently in emergency response situations.
  10. Assist Supports Coordinator to train, supervise and evaluate Supports staff.  Participate in hiring, retention, discipline and termination processes as needed.
  11. Participate and assist in conducting staff meetings.

 

A valid WI driver’s license, auto insurance, and the ability to meet Clarity Care’s driving standards are required.  Qualified candidates must possess a high school diploma, be at least 18 years of age.  Must be able to lift 50 pounds.  The ability to pass periodic criminal background checks under the Wisconsin Caregiver Law is required. Previous experience (1-3 years) working with individuals with developmental disabilities.  Training classes are required and provided by Clarity Care. The classes are held Monday-Friday between the hours of 8:00a-5:30p. All classes must be completed within your first 3 calendar months of hire.
