Employer Benefits Available to All Regular Full Time Employees

scheduled for at least 32 hours per week:

 

Health Insurance

Clarity care offers two different group carrier insurance plans on an individual, single plus 1 or family plan. Coverage includes company paid routine wellness benefit, along with a prescription drug plan. Clarity Care focuses on keeping employee cost down while offering great coverage and service.  

 

Dental Insurance

Clarity care offers a group carrier dental insurance plan on an individual, single plus 1 or family plan. You may select any dentist you wish, with additional savings by utilizing our dental network.

 

Group Term Life Insurance, Accidental Death and Dismemberment Insurance

This company paid benefit offers coverage at one times the employee’s annual salary.

 

Long Term Disability (LTD)

This company paid benefit is designed to work with other sources of disability income to replace part of the employee’s income if they become disabled as defined by the LTD plan. LTD is available only to Office/Administrative and Home Health Nursing employees.

 

Holiday Pay

Clarity Care offers Residential Managers, Coordinators, Office/Administrative and Home Health Nursing staff eight paid holidays off per year. Other full time staff receive time and one half pay for all scheduled hours worked on eight holidays per year.

 

Floating Holidays 

All full time employees will receive two floating holidays for the calendar year.

 

Vacation 

Vacation is earned monthly based on years of service.

 

Sick Time

Sick time is earned at the rate of four hours per calendar month of paid time. 

 

Jury Duty Leave

If a regular full time employee serves on a jury he/she will be excused from his/her scheduled hours of work to serve. Employees will be reimbursed the difference for their time missed during their shift upon receipt of a copy of the check for employee’s jury duty fees for a maximum of fourteen days per trial.

 

Funeral Leave  

Full time employees are entitled to paid funeral time, with a maximum of three days paid for the preparation for and attendance at the funeral of the employee’s immediate family member. One paid day is available for attendance at the funeral of certain other family members as defined in the employee handbook. 

 

Employer Assisted Homeownership Program

If you are not a homeowner already, this benefit is for you!  Clarity Care is offering a $3000 forgivable loan to our employees to help with the purchase/down payment of a home. Additional funding of up to $16,500 (funding will vary by county) may also be available to help with the purchase of a home. Employer Benefits Available to All Regular Full Time and Part Time Employees scheduled for at least 20 hours per week:

 

Flexible Spending Accounts

Employees may contribute to the plan, and then reimburse themselves for eligible health and dependent care expenses. Participation in the plan reduces taxable income, saving employees federal income tax.

 

Retirement Plan

Employees may invest a percentage of their gross salary into Clarity Care’s 403-b Retirement Plan. There is an annual discretionary employer match. The deferral decreases their taxable income.

Employee Assistance Program

Clarity Care firmly believes that the most important and valuable resource it possesses is the talent and dedication of its employees. As a result Clarity Care offers a confidential employee assistance and referral program to assist the employees and their immediate family members in the handling of everyday responsibilities, coping with life’s problems and dealing with unexpected crises.  

 

Direct Deposit 

An employee’s payroll check is deposited into a financial institution as directed by the employee.

 

Wellness Program

Clarity Care and the company sponsored Wellness Committee is committed to empowering our employees to achieve and maintain optimal health. This empowerment will evolve through continuous education and communication of wellness resources and opportunities available to all Clarity Care employees. In exchange for employee participation in specific wellness activities Clarity Care will provide wellness incentives. Health Risk Assessments are offered annually to all employees through our Wellness Program.

 

Achievement of Excellence Award

The Clarity Care Employee Achievement Award of Excellence was established to recognize outstanding employee contributions and exceptional performance. Employees are entered for consideration via nominations. Any Clarity Care employee may nominate a co-worker, supervisor or any other employee.  All regular full-time and regular part-time are eligible to be nominated.

 

Gladys Ihde Scholarship

Special Education majors or students with a Human Services (Human Development, Nursing etc) major intending  to work with persons with disabilities, who are at least in the second semester of their sophomore year (at time of application) are eligible to apply for the Gladys Ihde Scholarship. Ms. Ihde was an Oshkosh educator who left an endowment to Clarity Care to assist students. Preference will be given to Clarity Care employees in good standing or children of employees who demonstrate financial need, who have shown strong academic performance and dedication to work with children and adults with developmental disabilities.

 

Service Awards 

It is important to Clarity Care to recognize the contribution and commitment of each employee. The Service Award Program is designed to recognize employees who reach various levels of service with the company. The program provides a range of personal gifts depending upon the service award level.

Family/Medical Leave

Clarity Care provides the same leave to eligible employees as required by federal and state Family/Medical Leave laws.
