Our Department Heads


Barb Salemi 
Chief Executive Officer

Barb Salemi is the Chief Executive Officer (CEO). She joined Clarity Care in June 2008 as Chief Financial Officer (CFO), and accepted the position of CEO in July, 2015. Barb oversees a team of executive leadership to ensure the delivery of quality care and services to nearly 1,500 consumers each year. Under Barb's leadership, Clarity Care will continue to expand its service reach and scope.


Barb holds an accounting degree from DePaul Univerisity. Barb is a graduate of the 2010-2011 Leadership Oshkosh class.


Barb is originally from Northern Illinois, and is happy to call Omro home. She enjoys decorating, cooking, Sudoku puzzles, water-based recreational activities and spending time with family and friends.





Jeff Stark
Vice President of Facilities and Procurement

Jeff Stark is Vice President of Facilities and Procurement and has been employed at Clarity Care since 1992. Prior to becoming Vice President of Facilities and Procurement, Jeff worked as Director of Facilities, a maintenance team member, and a trainer for the Wheelchair Transport Training and Vehicle Safety classes.


Jeff's current responsibilities include leading procurement functions focused on strategic sourcing, logistics and efficiency, as well as the physical maintenance of all Clarity Care facilities and its fleet of vehicles. Jeff is a graduate of Leadership Oshkosh class of 2008.

Jeff lives in Oshkosh with his wife, Ann. They have three daughters who reside in Sheboygan, Grafton, and Chicago, and four grandchildren. He is a member of the Oshkosh Area Community Band, with whom he plays the trombone.  In his free time, Jeff enjoys woodworking as a hobby.



Heather Petersen

Chief Financial Officer

Heather Petersen is the Chief Financial Officer (CFO). Heather joined Clarity Care in 2005 as an Accounts Payable Clerk, and has held several positions including Financial Analyst and most recently Director of Business Administration. Her current responsibilities include leading, supervising, and managing the various aspects of Clarity Care’s Accounting and Recruiting Departments, as well as promoting and encouraging the organization’s mission, vision, values, and philosophy.
Heather has a Bachelor’s Degree in Accounting and Business Management from Pensacola Christian College. She resides in Oshkosh with her husband Michael, and is very active in her church. She enjoys travel, mentoring youth in the community, as well as spending time with family and friends.




Stacy Danford

Director of Quality Assurance (DQA)

Stacy Danford is the Director of Quality Assurance (DQA).  She joined the Clarity Care team in February 2008.

Prior to becoming the DQA, Stacy proudly held positions as a residential Coordinator, Coordination Team Leader and the Supportive Homecare Outreach Coordinator.

Stacy’s current responsibilities include overseeing the quality improvement of various Clarity Care programs and regulatory compliance.  Her goal is to create continuity, ensure quality and safety of services delivered in compliance with Managed Care, County, State and Federal laws and regulations as well as organization policies.

Stacy was born in Oconto and moved to Green Bay at age 21 where she still currently resides.   Stacy adores spending the majority of her free time with family and friends.  Stacy enjoys music concerts, traveling the world and watching the Packers.  She is very passionate about all animals and spoils her little dog, two cats and fish.




Rayne Nelson

Director of Residential Services

Rayne Nelson is the Director of Residential Services. She joined Clarity Care in 2002 as a Residential Coordinator and has since held positions including Coordination Team Leader, instructor, and Division Administrator.


Rayne’s current responsibilities include leading and overseeing Clarity Care’s team of employees responsible for delivering individualized services in community based homes for adults with disabilities and chronic conditions. She promotes quality services aligning with Clarity Care’s mission and in compliance with regulatory agencies.


Rayne is originally from Rhinelander WI and moved to Green Bay in 2002. She currently resides in Greenleaf with her husband, Jason, and children. She enjoys time and activities with her family, as well as being ‘up north’ spending time outdoors.





Holly Schlag

Home Care Administrator and RN Nursing Supervisor

Holly Schlag is the Home Care Administrator and RN Nursing Supervisor. Holly joined the Clarity Care team 2003. Holly started out in as direct support provider in the residential department; Holly served many positions in the residential division such as direct support provider, Lead Direct support provider and Coordinator. Upon completion of her LPN, she moved into the Home Care Department in 2010, starting with the scheduler position, then promoted to direct care supervisor until obtained RN degree and was then promoted to Home Care Administrator and Nursing Supervisor.


Holly was born and raised in De Pere. She obtained her Associate Degree in Nursing from Northeast Wisconsin Technical College in 2014. She currently is attending University of Wisconsin – Green Bay and plans to graduate with her Bachelor’s in nursing December 2017.


She is married and her and her husband share three children and enjoy camping and attending sporting events and spending their times outside.
